I saw a copy of this in our library at work and thought it would be a useful aid so bought the Kindle version. When it comes to care planning around Risk, Mental State, Section 17, and so on physical care can easily be overlooked. There's stuff in here that is extremely helpful if your trying to be as holistic as possible. Recommended.
